The Michigan Department of Health and Human Services Housing and Homeless Services Section has issued a Request for Proposal to provide services to homeless individuals. The goal of PATH is to reduce or eliminate homelessness for individuals with serious mental illness or co-occurring serious mental illness and substance use disorder. PATH funds are used to provide services including engagement and outreach, case management and housing. Eligible applicants include nonprofit organizations and local public entities. The PATH initiative project period is Oct. 1, 2018 – Sept. 30, 2019
